The effect of pharmacist centered medication reconciliations on transitions of care in type 2 diabetic patients – a focus on quality
Background: Ineffective transitions of care are a major problem in health care as it results in an increase of medication errors due to inadequate communication amongst health care providers. Miscommunication can result in avoidable hospital admissions, prolonged hospital stays, and increased health expenditures. Throughout the medication reconciliation process, health care professionals can reduce medication errors by partnering with patients to ensure accurate and complete medication information transfer at interfaces of care.
