Superficial resection combined with photodynamic therapy for successful treatment of facial lupus vulgaris with squamous cell carcinoma

A 65  year-old female presented with right facial patches for 30 years and a lump for half a year with slight itch and pain. She had accepted irregular topical steroid and antibiotic treatment in local hospital. Dermatological examination revealed a large area of dark red patches in the right face, w ith a 4cm × 3cm × 1 cm lump and blood scab on the surface (Fig. 1). The patient denied any history of other diseases. Pathology examination showed excessive epidermal cornification, mild acanthosis hyperplasia, partial ulcer formation, diffuse squamous-cell mass infiltration in dermal su perficial layer, distinct cell heteromorphism, as well as lymphocyte and plasmocyte-based inflammatory cell infiltration in dermal deep layer, with visible epithelioid cell granuloma and scattered multinucleated giant cells in partial regions.
