Implementing the medicines reconciliation tool in practice: challenges and opportunities for pharmacists in Kuwait

Medications errors and discrepancies which occur during patients ’ transition between different healthcare settings can result in serious harm to patients and substantial healthcare costs [1–3]. These errors occur most commonly at interfaces of care, particularly at the time of hospital admission due to incomplete medication history acquisition [4–6]. Medic ation errors are also likely to occur at the time of patient discharge from hospitals as a result of discrepancies in medication information or patient discharge with inadequate medication instructions [3,7].
