Integrated care in Switzerland: Results from the first nationwide survey

Socio-economic and technological advances contribute to increased life-expectancy and population ageing, which impacts on chronic conditions ’ prevalence and puts health systems under pressure worldwide [1,2]. Numerous challenges have been identified such as misfits between systems designed to deal with acute health problems and increased needs for chronic diseases management, resource’s shortages (financial, human), interinstitution al and interprofessional fragmentation, lack of care coordination as well as primary care weaknesses [1,3–5].
