Peripheral arterial disease (PAD) is gaining attention as a significant part of the atherosclerosis epidemic1 although there remains a lack of high-quality epidemiological data for distal occlusive disease.2 Particularly lacking are robust data on what has been referred to as critical limb ischemia (CLI). This is because studies are difficult with many patients lost to follow-up and the exclusion of patients with diabetes if the criterion for CLI is correctly applied. In addition, episodes of care for CLI can be assigned to other conditions under the International Classification of Diseases system.
