Heart failure (HF) is a major health care burden in older adults (Mozaffarian et al., 2016) and the leading cause of hospital readmission in the U.S. population (Medicare Payment Advisory Commission, 2016). In the United States, the Centers for Medicare& Medicaid Services developed the Hospital Readmission Reduction Program (HRRP) to decrease readmission rates (Schwartz et al., 2014).
