Supporting Transitions in Care for Older Adults With Type 2 Diabetes Mellitus and Obesity

Currently, 30.2 million adults 18 and older in the United States have diabetes (Centers for Disease Control and Prevention [CDC], 2017). The total prevalence of diabetes is projected to increase to 25% to 28% of the U.S. adult population by 2050 (Boyle, Thompson, Gregg, Barker,& Williamson, 2010). This is a 12.8% to 15.8% increase from the current prevalence rate of 12.2%. Type 2 diabetes mellitus (T2DM) accounts for approximately 95% of all diagnosed diabetes cases in adults.
