Pathogenesis of Idiopathic retinal vasculitis, aneurysms, and neuroretinitis (IRVAN) or ‘Idiopathic retinal arteriolar aneurysms (IRAA)’ with macular star

I read with interest the interesting hypothesis regarding the pathogenesis of the so-called ‘idiopathic retinal vasculitis, aneurysms and neuroretinitis (IRVAN).[1] The authors suggest that in IRVAN, there is no true inflammation of retinal arteries. They propose that ‘a developmental or congenital presence of saccular macroaneurysms’ leads to ‘vascular flow abnormalities’ result ing in retinal capillary nonperfusion (CNP) and generation of vascular endothelial growth factor (VEGF). The angiographical paravscular staining has been hypothesized by the authors to be ‘a result of the VEGF produced by the adjacent CNP areas’.
