Safe Transitions for High Risk Patients

From the Agency for Healthcare Research and Quality (AHRQ): “Transitions of care among ambulatory sites are vulnerable to patient safety gaps. Patients who transition from one ambulatory care facility clinician to another are especially vulnerable to patient safety errors. This is due, in part, to a lack of effective communication and patient engagement in shared decision making.” The AHRQ has made available a toolkit for medical staff who are responsible for patients during transitions of care. The toolkit offers ways to help engage patients and their caregivers in their care to help prevent errors and includes PowerPoint slides, as assessment, checklists and an appointment aid.
