HIV Care Continuum Manuals

The Health Resources and Services Administration (HRSA) developed four manuals to help HIV focused organizations implement evidence based interventions. These manuals focus on specific populations and highlight strategies such as cross-agency coordination. The manual Transitional Care Coordination from Jail Intake to Community HIV Primary Care, can be used by clinics, health departments, and jails to better connect health care systems within jails to community healthcare and ensure that those leaving jail have appropriate support. Integrated Buprenorphine Treatment in Opioid Use Disorder in HIV Primary Care is geared toward HIV primary care providers and focuses on the need to treat opioid use disorder in those living with HIV to promote greater quality of life. Enhanced Patient Navigation for HIV Positive Women of Color is a manual help organizations, agencies and clinics use patient navigators to reduce barriers to care. Peer linkage and reengagement of HIV Positive Women of Color can also be used by organizations, agencies and clinics to use a peer support model to improve health outcomes in this population. An additional resource for those working with individuals living with HIV is AIDSinfo, which offers information for healthcare providers, researchers and the general public about HIV/AIDS practice guidelines, prevention and treatment.
