Rural Aging in Place Tool Kit
Aging in Place, or remaining in their homes, is something many older Americans would like to be able to do. Planning ahead and knowing how to access services can help make this possible. The Aging in Place Toolkit for rural populations has been updated by the Rural Health Information Hub. The toolkit compiles evidence-based and models and resources to support organizations which are implementing aging in place in rural communities across the United States. The modules in the toolkit contain resources and information focused on developing, implementing, evaluating, and sustaining rural aging in place programs.
