Retour d ’expérience : un an de télé-expertise en dermatologie libérale en Corse

The objective of this study was to assess the effectiveness and the relevance of telemedicine usage for chronic wounds and skin tumors in private practice, taking into account the medical demography, the aging of the population and the geographical remoteness in Corsica of a dermatologist. Material and methods Recruitment of stakeholders (general practitioners and dermatology experts). A telemedicine contract was drawn up and signed with the regional Health Agency of Corsica by the end of 2014. The physicians involved signed an agreement and attended dedicated training. The experiment started in March 2015. Setting up of clinical records and reporting for the completion of the tele-expertise; smartphone, tablet, digital camera, computer and secure messaging (MS Santé). Selection of relevant indicators to build the activity follow-up table (collection and analysis of data). Communication to patients and physicians (poster, press interviews). Results From March 2015 to March 2016, recruitment of 42 general practitioners and 6 dermatologists. A total of 20 indicators have been informed and 130 patients were involved in the programme; many applications came from northern Corsica, the area most lacking in dermatologists. Fifteen confirmed cases of cancers, mainly basal cell carcinomas, 18 tele-expertises were performed for chronic wounds with 4 wounds completely healed.
