Few questions - Help us improve communication with patients with cardiovascular (heart attack) problems.

Dear Cardiologists, I know how valuable your time is so I will keep it as concise as possible. We are a group of young entrepreneurs, currently taking part in Harvard Business School program "Innovating in Heathcare" and under its auspices we are trying to solve the problem of miscommunication between patients and cardiologists. According to various publications and researches, despite multiple tools and efforts to convey information to patients in an understandable way, there are still significant problems with post-appointment compliance resulting in readmissions and complications what costs billions of dollars and jeopardizes patients' life. We are currenty trying to analyze the problem in depth and we are turning to you as we need help and feedback from the people who are in the frontlines. Therefore, it would be great if you could help by briefly answering/addresssing the following questions.  - Which part of adherence is the most important for patients who suffered a heart attack? - Which part of adherence do those people fail most often? - Who on staff is iultimately responsible for relaying recommendations and instructions? - How do you currently explain the diagnosis and recomendations to your patients and to what level of detail? - How do you know if the patient understands the information and instructions? - Do you and how identify illiterate patients who may be uneducated or do not speak English fluently enough? - How is the waiting ...
