Discussion et conclusions Les taux de transferts d’hospitalisations ne permettent pas dans notre étude d’être améliorés grâce à l’utilisation de Trajectoire. Néanmoins, le logiciel permet de transmettre des informations médicales et sociales de qualité entre les services et il contribue donc à l’amélioration des prises en charges médicales dans leur globalité. Le délai moyen de réponse très court des unités d’accueil montre tout de même que Trajectoire a une influence sur la diminution de la DMS. Introduction To optimize the management of cancer patients, an efficient and easy organization of hospital discharge is required, especially transfers from health care facilities to institutions that provide follow-up care and rehabilitation, long-term care or palliative care unit. In this context, since June 2012, the French National Authority for Health (HAS) software Trajectoire has been used as an interface in our hospital, the Oscar-Lambret center (Lille, France).
