Publication date: Available online 25 July 2016 Source:Pediatria Polska Author(s): Maryna Krawczuk-Rybak Systematic improvement of treatment of neoplastic diseases in childhood resulted in a growing population of survivors which are threatened by a number of late effects of therapy. The possibility of organ damage, which can occur even after several years off the therapy, is known to paediatric oncologists, but not always to other specialists and primary care physicians, and neither to the patients. A major problem is the transition of young cancer survivors from paediatric to adult multidisciplinary care. Adolescent or young adult is faced with taking responsibility for their own health and the need for self-control, which requires knowledge about disease, treatment and the possible long-term effects. This process of transition must be gradual, needs to be initiated early, with the systematic transition plan and education on the diagnosis, treatment and possible consequences or risk factors of treatment, and sometimes the possibility of prevention. The survivorship ‘passport’ should be given at the end of treatment and should be available to general practitioners and other specialists, under whose care the patient will be treated. Paediatric oncologists have to create a program of active transition of adolescents/young adults to adult health care, which will systematically broad their knowledge, awareness and responsibility.
