Teaching Transitions of Care: Finding New Solutions to an Old Problem (Darlene Moyer MD)

In 2013 the ACGME implemented Institutional and Common Program requirements related to transitions of care. Since that time, the HonorHealth Family Medicine Residency Program has made changes to our inpatient clinical assignment schedules, has designed and implemented a Transition of Care longitudinal curriculum, and has adapted hand-over processes to ensure better continuity of care and patient safety. This presentation will review the ACGME requirements applicable to transitions of care and describe the longitudinal curriculum and training program that we have implemented. This will be an interactive session where the audience can participate in transitions of care-related activities as well as share ideas from their own programs so that we can learn what others are doing to meet this requirement. We will discuss some of the challenges that we have had related to implementing these changes at our program and engage the audience with some ways to address transitions of care in their training environments moving forward.
