Introduction Stroke is a therapeutic emergency that requires prompt care in a specialized stroke unit (SSU) under the supervision of a neurology specialist. There are only two SSUs in Burgundy (France). That is why, in order to reduce regional inequalities and to promote uniformity in access to healthcare, telemedicine and fibrinolysis thanks to telemedicine were developed in Burgundy within the Telestroke system. Nevertheless, the introduction of this new health plan has a cost. Materials and methods All patients presenting an ischemic stroke confirmed by cerebral imaging between the 1st January 2014 and 31st June 2014 and who underwent fibrinolysis (with or without telecare) were included in this retrospective multicenter observational study. It aims to assess accurately the differential price between patients who received fibrinolysis in an SSU (Dijon) and those who received care in a spoke center (Auxerre, Beaune or Semur-en-Auxois). Expenditure has been evaluated by using microcosting technique. Results Our sample is made up of 10 patients who received fibrinolysis via telemedicine and of 8 patients who received fibrinolysis in an SSU.
