Some patients unresponsive to conventional, first-line therapeutics develop recurrent, recalcitrant ENL. Here, we reported a case of severe refractory ENL that was successfully treated with Etanercept. Biologics may be considered as therapeutic alternatives in management of severe, recalcitrant ENL.
