Tranexamic acid for prevention PPH: a promising drug but today only a promising drug

We congratulate Alam and Choi for their excellent systematic review regarding the use of tranexamic acid (TXA) to prevent postpartum blood loss and postpartum hemorrhage (PPH) after both cesarean and vaginal deliveries [1]. We would like in particular to congratulate the authors for their main caution take-home message and their excellent critical analysis of the quality of the available evidence: more evidences are necessary before implementing TXA in routine practice for all deliveries. As we previously highlighted in a non-systematic review [2], Alam and Choi have underlined the weakness of the existing literature as are the data about its benefit-harm ratio in order to amend their results suggesting that TXA is effective to improve maternal health [1].
