What immunosuppression should be used for old-to-old recipients?

Transplant centers are faced with an increasing number of elderly patients on the waiting list for renal transplantation. For example, in France, the number of patients over the age of 60 years on the waiting list has multiplied 4-fold in the last ten years and, today, represents 38 % of the patients who are listed and a third of the patients who have had transplants [1]. This is a result of the aging of the End Stage Renal Disease (ESRD) population and is also a consequence of the expected benefit for the recipients, explaining why advanced age is no longer considered a contraindication to transplantation.
