Patient Discharge ‘Time Out’ Reduces Medication Errors

June 27, 2015 - When a patient is ready to be discharged, a nurse reviews the medications that the patient is currently taking and will need to take after he or she leaves. Ideally, this medication reconciliation process will go smoothly--and there will be no discrepancies. Of course, in the real world, discrepancies are often detected during this process, and some aren’t picked up at all.
