Cysts arising from Echinococcus granulosis occur most commonly in the liver and secondly in the lungs. Primary cystic involvement in the musculoskeletal system without lung or liver involvement is very rare. Treatment consists of surgical resection. Anthelmintic medication before and after surgery can be used. In this article, we present a very rare case of 29-year-old female with hydatid cyst involving shoulder, starting from subcutaneous tissue and extending up to the deltoid muscle.
